The Electroporation Treatment
Mirai Medical hase developed ePore: an innovative low energy highly tolereted Electroporation system.
The ePore electroporation treatment can be used alone or in combination with specific chemotherapy drugs or agents (complying with the standard operating procedures for Electrochemotherapy) and allows to manage the treatment of skin cancer lesions such as:
  • Melanoma metastes, single and in transit 
  • Breast cancer metastasis
  • Basal and Squamous Cell Carcinoma
  • Head and Neck
The effectiveness of electroporation in skin tumours ablation clinically has been reported by a growing deal of clinical studies with an over 80% objective response rate observed.
This new electroporation treatment approach is brought directly to the target by an ergonomic handle needle electrode array. The electroporation energy is delivered via a patented new painless waveform generated by the ePore device inducing cells to become porous or leaky for several minutes.
Specifically, the clinical values of the electroporation treatment are:

  • Efficacy. Electroporation treatment in combination with chemotherapy has been investigated in a controlled manner, in a multicentre, prospective international clinical trials, demonstrating a complete tumour response of with a single treatment and an objective response of 85%.
  • Haemorrhagic nodules stop bleeding immediately after electroporation treatment.
  • Long-lasting response. Electroporation induces a high percentage of long-lasting local remission.
  • Improves patients’ quality of life: wound healing, wound bleeding, aesthetics, daily activity, better social relations, pain relief, overall satisfaction and acceptance of the treatment.
The Electroporation Procedure
Mirai Medical has created unique waveforms to deliver a pain-free electroporation treatment.
A local anaesthetic is first applied to the skin and  the ergonomically designed probe is adjusted via a simple twisting mechanism to modulate the needle length to the required depth (5 – 20 mm).
Once inserted into the skin, the ePore device delivers the electroporation pulses by means of touch screen interface. Selectable options on the ePore touch screen allow for use as standard  electroporation, pain free electroporation.

Generator ePORE
The ePORE generator is unique in being capable  of delivering electroporation pulses at a frequency of 250kHz eliminating muscular contractions and allows the procedure to be performed under local anaesthetic (LA). 
This advancement means that many patients will be treated under local anaesthetic in an out-patient setting, reducing the time and financial burden on healthcare resources, the treatment of patients earlier in their disease state, and the ability to offer treatment to patients that are unable to tolerate general anaesthesia. 
This significant innovation in electroporation delivery has stemmed from multiple years of R&D and a team that has been jointly involved with electroporation field for more than 20 years.

CUTIS electrode
MIRAI MEDICAL has developed the CUTIS electrode to target cutaneous and subcutaneous cancer lesions. The adjustable length electrode can deliver electroporation pulses to tissue up to 20 mm deep.  Taking feedback from experienced clinicians across Europe, CUTIS skin electrode has been designed and engineered with both the clinician and patient in mind.

The retractable head offers unique benefits:

  • 1. Needles are fully supported as they enter and penetrate tissue, avoiding needle bending and convergence.
  • 2. Patients treated under LA do not see the needles, relieving potential anxiety.
  • 3. Safe - no need for recapping.

The future of ePORE and Electroporation
The combination of electroporation and calcium is also an extremely promising area for use with electroporation. Falk et al4 found in a double blinded randomised phase II study in patients with cutaneous metastases that calcium combined with electroporation is safe and has a similar response rate to bleomycin.
The combination of high frequency electroporation performed under local anaesthetic in an out patient setting opens the door
to the use of electroporation earlier in the patients disease state, including patients with premalignant disease and those with benign lesions such as keloid scars.
